GUIDE TO THE SYSTEM
Health-care institutions
experience some of the most serious medical diagnostic problems and consume the
highest volume of expenses within the public health care system; furthermore,
they have a key role in public medical education and technological advancement
as they concentrate considerable material and human resources.
Implementing health reform
within this sphere is a serious challenge for managerial and clinical teams in
terms of organizational, informational and managerial tasks to be completed
under shortage of funds and of technical, technological and human resources. The
management of Bulgarian hospitals today faces an acutely growing new “aspect” of
the hospital function – to secure financing for its existence and development.
For several years now the ITM
company (Informatica Telematica Management) – Ruse has been successfully
distributing its integrated automated system IAS “HOSPITAL – XXI” as an integral
health management model for health-care institutions of the 21st
century. Our company has become the national leader in the implementation of
modern integrated management technologies thanks to our acknowledged pack of
integrated systems including IAS “Firm – XXI”, IAS “TEMIDA – XXI”, IAS
“MUNICIPALITY-XXI”, etc.
The system has been based on
some 40 years of extended experience in the automation of management activities
including management in the public health care sphere. The health care systems
and databases of a number of European countries have been thoroughly studied in
order to secure the compatibility and comparability of the factors between
European systems and our one. IT has been created in cooperation with
specialists experienced in modern diagnostic and therapeutic practices.
IAS “HOSPITAL – XXI” is far more
than a hospital information system, it is a hospital management system, applying
contemporary methods and instruments of management in the specific medium of
“hospital production”. It is, therefore, a health management system in the full
meaning of the term. This is the first EPR system within public health care
sphere which unites the possibilities of EPR business management systems and the
specificity of hospital management. It spans over the complete management
process in a hospital – the hospital and clinical units managers have
round-the-clock access to information about the indices, technology and
decision-making options. Up-to-date system architecture and information,
management and communication technologies are applied in the system. It
integrates the three major aspects of health management in hospitals, namely:

Furthermore, external
communication is also provided with the Ministry of Health, the National
Health-Insurance Agency, Municipality, etc. Each of these aspects has been

complex and comprehensive analyses of activities, costs and quality.
The system is successfully
applicable in all health-care institutions regardless of their ownership status
and funding sources.
IAS “HOSPITAL – XXI” implies the
philosophy, as much as the organizational and management technology of a

Operates a fully closed cycle of clinical
management by providing a feedback with the pre- and post-hospital care and
rehabilitation units; |

medical-diagnostic processes, financial-economic and management activities and

Combines the planning of
activities and the distribution of resources with effective control of the
costs and the quality of hospital services at all levels – starting from the
specific diagnostic – medical process on to the individual physical and
statistical patient and on to the specific disease (Medical Code of Disease)
and a group of diseases(GD), and further to the hospital team and the hospital
unit, and finally applying factor and comparative analyses for the hospital as
a whole, reaching to every participant in the production process and his/her

Maintains a detailed health file for the
hospital treatment of each patient from the date of his/her first attendance
to the hospital, and calculates the actual cost of treatment (bill for the
stay), submits the file to the pre-hospital units who have directed the
patient for hospital treatment immediately before and after the treatment
process. |

Completely document-free – operates with
electronic documents, optimizes the number and type of the documents and the
document circulation; ‘on the spot’ (at the register desk, patient reception
hospital units and consulting rooms, hospital wards, pharmacy, storehouses,
kitchen, cash desk, paraclinical units, etc.) all documents (initial,
secondary, chronological); the system contributes for diminishing the volume
of document processing by medical staff by eliminating the doubling of
information and by automating all info checks, reports, statistical records,
etc. It stores for unlimited time periods all documents and provides immediate
access to them; |

Developed in strict compliance with the
acting legislative and regulatory documents and requirements; it uses approved
primary documents specified by the Ministry of Health, the Ministry of
Finance, the National Social Security Institute, the National Statistical
Institute as well as the relevant forms with coding corresponding to the
adopted international and national classifiers; it provides in the needed form
and content, periodicity and carrier practically every document, such as
reports, information records, statistical records, specifications, etc. |

Provides new opportunities and options to
hospital managers and clinical staff, such as marketing, logistics, budgeting,
comparative and case-mix analyses, prognoses, etc. |

Object-designed and object-programmed to
provide optimizing of the integrated database (access to the database and
organization of the procedures generating info checks, reports and analyses at
random levels). The database contains the 14 adopted basic indices in the
integrated system (IS) as well as some additional indices for quality
management, thus ensuring the availability of one of the main quality
management instruments – comparability of hospital work results with the set
standards. The database can be enlarged and used by hospital specialists for

Shows for each health index the current
(moment) value, the value for a set past period of time by months, quarterly
periods and years; comparative analysis of the index in terms of an average
value for the whole period and outliers in sub-periods, an average value for
the country, for the type or volume of the health-care institution, for the IS
by sub-periods and outliers, CASE MIX analysis and Outliers, etc. |

Has a modern financial-accounting system
focusing on the technology of management of the specific hospital ‘production’
which functions fully on documental basis and in real time. The documental
nature of the system provides management information at the moment of creating
the documents, independently and apart from the process of accountancy
structuring and terminology, and accounting operations; |

Contains models for decision-making, analyses
and prognoses in the form of written technology for management of activities,
procedures and operations concerning the staff. |

Menu-oriented, with an in-built menu
generator, allowing free definition of a menu for each employee or a group of
positions within a desired structure, content and regulations of activity
which allows optimizing the distribution of the system without functional and
documental doubling; |

Features an ergonomic, user-friendly and
intelligent interface for end users, based on natural actions and dialogues
involved in documentation processing, allows working with object designations
rather than with object codes or ciphers, in their natural structures, and
provides options for search by multiple properties: text, Personal number (EGN),
IZ, first names, ciphers according to national and international
classifiers, etc. This type of interface allows end users to attain
proficiency in operating the system in just a few days; |

Has a special system access control to
prevent unauthorized access and demolition – up to 6 levels; a special access
mode is provided to the patients’ personal and medical treatment information,
and their health files; |

Offers a pack of administrating instruments,
applicable also for the database and accessible only by the system
administrator; |

Features high adaptability and allows
adjusting the system to the specific structure and work organization of any
hospital, to meet amendments in the legislative and regulatory norms;

Does not require establishing of specialized
informational departments or of coding operator teams for inputting packs of
accountancy information. |
